We at Lifeline are proud to announce we have commenced our trial service to return people well enough to return home after a spell in hospital or respite care. The Service consists of our dedicated team of call handlers receiving information on the individual requiring take home.
Once a risk assessment has been completed we agree a time and date to pick up that individual and transport them home the their comforts and familiar surroundings.
We ensure that the individual has enough supplies to get through the first day or two for essentials such as tea, coffee and milk etc. a calling card is also left in case of emergencies whilst awaiting any follow up input from other voluntary or NHS sectors working collaboratively ensuring independence to those using the service.
